The undersigned, for themselves and their minor child(ren), listed below, hereby acknowledges that they are participating in an event at a working farm, and agrees to abide by the rules, conditions, and restrictions contained herein, and where applicable, to ensure that their minor child(ren) will do the same.

During the course of the workshops, classes, field trips, events, tours, or activities taking place on the farm, participants may be exposed to certain hazards and dangers, both known and unknown.  Such hazards include, but are not limited to, animals that may bite, kick, or jump up, operating equipment and heavy machinery, wild plants that may be toxic (including poison oak, sumac, parsnip, or ivy), stinging insects, ticks, and sharp objects.  The undesigned hereby acknowledges and accepts such risks on behalf of themselves and their minor child(ren), and, in cases where the child(ren) is accompanied by the undersigned, the child(ren) shall be supervised to ensure compliance with the terms herein.

For the safety of the attendees and animals, the undersigned, for themselves and their child(ren), hereby acknowledges the following rules:

DO NOT FEED ANY ANIMAL WITHOUT BEING GIVEN PERMISSION BY AN AGENT OF PROMISELAND FARM LLC

DO NOT ATTEMPT TO FEED ANY HORSE ON THE PROPERTY OR ATTEMPT TO TOUCH THE HORSES WITHOUT THE SUPERVISION AND PERMISSION OF AN AGENT OF PROMISELAND FARM LLC

DO NOT PICK OR EAT ANYTHING FROM THE FARM WITHOUT BEING GIVEN PERMISSION.

DO NOT LEAVE CHILDREN UNSUPERVISED ON ANY FAMILY FARM TOUR.

DO NOT TOUCH OR USE ANY TOOL OR EQUIPMENT WITHOUT PERMISSION FROM AN AGENT OF PROMISELAND FARM LLC

REPORT ANY INJURY OR ISSUE TO AN AGENT OF PROMISELAND FARM LLC

 IMMEDIATELY.

In addition to the above, the undersigned hereby forever holds harmless and waives any and all claims, both known and unknown, against Promiseland Farm LLC., its agents, officers, employees, or assigns, as well as the property owners of the farm, Matt and Kellyann Harmon, for any accident, injury, claim, liability, casualty, or other claim arising from or in any way related to the farm, fieldtrip, tour, class, workshop or other event hosted by Promiseland Farm LLC.  You are undertaking this activity at your own risk.  

I hereby acknowledge that certain farm fresh produce or other snacks may be made available to either myself and/or my minor child(ren) while attending an event with Promiseland Farm LLC.  The following is a list of critical medical information that is relevant to our personal safety:

I further acknowledge that I have read this form in its entirety, that I understand the contents herein, and that, on behalf of myself and my minor child(ren), I hereby agree and accept the terms herein and accept the risks associated with participation in events held on a working farm.

CONSENT FOR PHOTO AND MEDIA USE BY PROMISELAND FARM 

The undersigned, for themselves and their minor child(ren), listed herein, hereby acknowledges that during participation in the events contemplated above, there will, at times, be photographs and video taken.  Such media may be used by Promiseland Farm for the purposes of promotion and marketing of farm classes, tours, and events hosted by Promiseland Farm.  Such promotions may include mailers, website updates, and postings to social media accounts.  By signing below, I hereby authorize Promiseland Farm on behalf of myself and my minor child(ren) listed herein, to be photographed and recorded during their participation in Promiseland Farm activities and for such media to be used for the express purposes stated herein.  Promiseland Farm allows participants to take photos and video and Promiseland Farm cannot control the usage of any photographs or video taken by participants.  To that end, please be advised, that you or your child’s image may appear on private social media pages or other websites.
